TERMS & CONDITIONS

You are eligible for psychological treatment. Before going through this process, I ask you to agree to the conditions with which I work. Therefore it is important that you are aware of the following conditions:


- If the contact is unilaterally stopped by Arthur Hofman or is not continued after being introduced, referral to another practitioner or institution will be provided - if advised and desired.

- Commissioner/patient indicates by continuing the contact after first acquaintance with these terms and conditions that they have been brought to their attention, read and accept.

- Changes to these terms and conditions at the request of the commissioner/patient only apply if they have been signed in writing and signed by both parties.

- The patient is at all times, both inside and outside the practice, responsible for his or her own behavior and the consequences thereof. Arthur Hofman is in no way liable for any damage that might have been caused by the contact in the eyes of the commissioner/patient or third parties.

- The content of the conversations is confidential and is treated with the utmost care and discretion. Arthur Hofman has a duty of confidentiality, which means that third parties cannot be informed of what is discussed during the contact without the patient's explicit consent.

- The patient has the right to insight and/or a copy of his or her file, with the exception of the personal (work)notes of the practitioner. In the case of treatment of the relationship, one partner does not have to give permission for inspection.

- Arthur Hofman is not liable for inaccuracy or incompleteness in the data provided by or on behalf of the referrer/commissioner/patient.

- Arthur Hofman considers himself bound by the ethical rules of professional conduct. Acting in an ethically responsible manner within the professional practice means that the practitioner, in all activities falling within his professional practice, respects the value and dignity of his patient and his right to his own beliefs and/or religious beliefs and the lifestyle based on them.

- Complaints about the quality of the practitioner's actions must be reported as soon as possible, but in any case within 14 days, after the relevant contact moment.

- For individual patients the agreed costs are settled at the end of every session in cash or by bank application (for which you receive an invoice by e-mail).

- Appointments can be canceled free of charge up to 24 hours in advance by telephone or text message. In the event of late cancellation, 100% of the call rate will be charged.

- All disputes that may arise between Arthur Hofman and the commissioner/patient and that are not described in these terms and conditions will in the first place be resolved in good consultation - with or without the help of mediation. If no agreement can be reached, the dispute will be subject to the judgment of Dutch Law.
